The survey will remain open until Friday, July 31.
It offers residents the chance to rate Port Stephens Council's performance in different areas and to give feedback on areas for improvement.
"We're responsible for providing a wide range of facilities and services for and on behalf of our community," council general manager Wayne Wallis said.
"These facilities and services consume a large proportion of council's budget and residents are affected by how we do our work so it's important that we are getting it right or identifying where we can do better."
The online survey could be found at surveymonkey.com/r/PSCGEN2015 or through Port Stephens Council's website.
