Port Stephens police are continuing to make inquiries into the incident in which the 69 year old from Medowie lost his leg after being trapped under his vehicle in a Raymond Terrace supermarket carpark on August 19.
Police said the incident occurred when a vehicle reversed from a car park by the 87 year old man collided with a bollard causing the elderly driver to "accelerate harshly and collide" with the Medowie man who was packing groceries into the rear of his vehicle.
The Medowie man was knocked to the ground and pushed underneath his vehicle. Police said the impact of the collision pushed the Medowie man's vehicle about one metre from its original position.
Emergency services were called. Police officers attended the scene immediately.
"First aid was rendered to the man and the scene was secured," police said.
Newcastle Crime Scene officers attended. Port Stephens SES Unit was called to extract the Medowie man from under his vehicle.
He was stabilised at the scene and taken by ambulance to John Hunter Hospital in a serious condition.
"The man sustained serious, debilitating injuries requiring intubation and care in the John Hunter ICU," police said.
The elderly male driver was taken to Maitland Hospital for mandatory testing.
A question of fitness on the licence of the driver has been submitted to Roads and Maritime Service's medical unit. The unit will assess the driver's fitness to retain his drivers licence.
Inquiries are continuing.
